Advance Care Planning with People with Dementia
An Advanced Care Plan enables health and care professionals working alongside people living with dementia to spend time talking with them (and their family carers where appropriate) about their future care. This opportunity will be limited by the fact that at some point in their dementia journey, people with dementia won't meaningfully be able to contribute to that process, so it is important to have more - and earlier - conversations about their future wishes. This short video will help support you to feel more confident in having these conversations.
Please share this video with health and care professionals staff but please don’t share it on social media channels – we have consent from participants for it to be shared publicly but it is primarily intended to be used for the purposes of staff awareness raising and training.

Dementia Learning Lunches - Resource Material and access to the live recording from the day:
On Thursday 10 December 2020, Practices were invited to a 45 minute “lunch time learning” training session to offer information and advice about supporting people living with dementia and their families/carers.
The session included information about:
•GP referral routes to community dementia services
•Advice/support for professionals with (non-medical) queries to help you support people living with dementia
•Support for carers of people living with dementia
Run on behalf of Sheffield CCG jointly led by Dr Sarah Jones (clinical lead for the Dementia Strategy) and Natasha Wilson (Dementia Advice Service, Age UK Sheffield).

On 18th February 2021 delegates attended the Dementia Care: Managing Crisis lunchtime learning session.
This event covered:
o Reducing the risk of getting to crisis point
o Contingency plans, using ‘Special Notes’, good information sharing practice
o How health and social care can work together to manage crisis situations
o Support for family carers
o Providing a better experience for people in a crisis
